The 2016 CDC Guidelines and newly implemented CMS Guidelines (as of January 1, 2019) have recommended opioid dose reductions with clear delineation at certain morphine equivalent daily dose points. Managed care organizations and various states have implemented these recommendations as policy or regulation which has resulted in forced tapers. It is our belief that opioid taper patterns, techniques, and overall approach are quite variable and can result in poor patient outcomes.
